Annual Check-Ups

Just like your car or computer, your body requires regular maintenance to function well. Annual check-ups are a simple, yet effective way to stay on top of your health. Yearly exams focus on preventing disease and enhancing wellness. At Baptist Health Primary Care, annual check-ups create a foundation for good health. Our doctors and nurses take time to evaluate your physical health, discuss your concerns and answer questions. This professional head-to-toe assessment is a proactive tool for a healthy life.

Chronic Disease Management

Living with chronic disease is less stressful when you have a great healthcare team. Baptist Health Primary Care offers quality medical care, education and support, lab services and specialist referrals. Our team listens to your concerns, asks questions and gets acquainted with you as a patient and a person. They learn about your lifestyle, medical and family history. We believe this is essential to your quality of life and longevity.

Nutritional Assessments

Good nutrition is a building block for good health. To enhance your well-being, each Baptist Health Primary Care has nutritional services through dietitians. They offer nutritional counseling for many concerns, including: weight loss, prenatal nutrition and healthy eating to manage chronic diseases. Working with your primary care physician, the dietitian develops a customized nutritional plan to fit your needs and lifestyle.

Mental Health Program

Emotional health is integral to your overall well-being. That’s why every Baptist Health Primary Care team includes a social worker. Our social worker helps patients manage a variety of emotional and behavioral concerns. Maybe you need support to overcome depression or to recover from addiction. Perhaps you need coping skills to manage stress.

Benefits of integrating mental health into primary healthcare:

  • Mental and physical health problems are interwoven and treated holistically in primary care.
  • Primary care settings enhance accessibility to mental health services.
  • Delivering mental health services in primary care settings helps reduce stigma and discrimination.
  • Treating common mental disorders in primary care settings is cost-effective.
  • The majority of people with mental disorders treated in collaborative primary care have good outcomes, particularly when linked with specialty services.

Our social workers can provide brief supportive counseling in person and over the phone. Their services include linking patients to psychiatrists and psychotherapists; reviewing and reinforcing behavioral health interventions recommended by doctors and nurse practitioners, such as medication adherence; and reporting improvement or worsening of symptoms. They also have the training and expertise to address acute situations, such as crisis intervention, and provide primary care support.

Diabetes Program

The Diabetes Program was created to target one of the most common chronic diseases seen in the primary care setting – diabetes. According to the National Standards for Diabetic Self-management, Education and Support, “all people with Diabetes should participate in diabetes self-management education to facilitate the knowledge, skills and ability necessary for diabetes self-care.” Improved diabetes knowledge is associated with better self-care behaviors, lower HgbA1C, lower weight, improved quality of life, healthy coping and reduced healthcare costs.

The program is managed by a multidisciplinary team of doctors, dietitians, nurses, healthcare coaches, social workers and exercise physiologists. A class called Diabetes and You provides patients with the information they need to better manage their condition: targets and goals, self-monitoring of blood glucose, hyper- and hypoglycemia, sick-day management, medication and insulin management, potential long- and short-term complications, foot care, exercise and at-home workouts, diet and carb counting, effects of sugar and artificial sweeteners, the benefits of fiber, meal planning and cooking, portion sizes, sodium limitations, alcohol use, eating out and weight management. Over a six-month period, patients undergo personalized insulin management, blood sugar monitoring, medication management and adherence, laboratory tests, individualized meal planning, supermarket shopping, healthy cooking, coping strategies, behavior changes and goal setting.

Lab Services

To deliver efficient, effective medical care, Baptist Health Primary Care offers onsite lab services, including blood work, urine tests, strep cultures and other common health screenings. This allows your primary care physician or advanced practitioner to diagnose and treat most illnesses in one visit. You save time and money and feel better faster.
