West Kendall Baptist Hospital and Healthy West Kendall Partner with Alliance for Aging to launch Care Transitions Program

MIAMI, FL – August 5, 2020 – West Kendall Baptist Hospital, Healthy West Kendall and the Alliance for Aging have teamed up to launch the Care Transitions Program to provide home-based assistance for geriatric patients discharged from West Kendall Baptist Hospital. The goal of the program is to help vulnerable seniors recuperate in their own homes and prevent or delay nursing home placements or hospital readmissions. The Care Transitions Program provides help and support for qualifying seniors for up to 60 days, at no charge. Developed in response to the COVID-19 pandemic, the temporary program is available through December 2020 to 150 patients and funded by the CARES Act Provider Relief Fund from the U.S. Department of Health & Human Services.

“West Kendall Baptist Hospital and Healthy West Kendall are pleased to partner with the Alliance for Aging in caring for our patients upon discharge,” said Michelle Mejia, assistant vice president of Health Promotions & Strategic Partnerships at West Kendall Baptist Hospital. “This extra support, which consists of meals, transportation, housekeeping and emotional support, will help our patients and residents recover and return to restored health.”

The Care Transitions Program is offered to area residents age 60 and over who are identified during discharge from West Kendall Baptist Hospital as needing help at home to promote healing and recovery. Patients who live alone or have limited means for self-care and support may receive assistance with meals, personal care, homemaking, shopping and more. The necessary services will be determined by the patient’s care team and case manager and delivered by staff of the Alliance for Aging service providers.
