Patients can access Baptist Health South Miami Hospital by parking in the attached garage on SW 62 Place. There is also a shuttle service available from South Miami Metrorail station. Call 786-662-4199 to request service.

Please bring your photo ID and insurance card with you to every appointment as well any insurance forms you need completed. Our welcoming primary care team also asks that you have your preferred pharmacy information with you, including name, address and phone number for easier registration.

Preparing You For Your Visit

To prepare for your visit, please make sure to bring the following items with you:

  • Photo identification
  • Insurance card
  • Copies of any relevant medical records, radiology images, consultation reports, etc.
  • Name of pharmacy, full address, and phone number, in the event that a prescription is ordered

    Parking is available in the garage adjacent to the facility. In addition, valet parking is available at the main entrance of the building.
