Preparing You For Your Visit

To prepare for your visit, please make sure to bring the following items with you:

  • Photo identification
  • Insurance card
  • Copies of any relevant medical records, radiology images, consultation reports, etc.
  • Name of pharmacy, full address, and phone number, in the event that a prescription is ordered
  • Parking

    Parking is available in the lot in front of the facility. In addition, valet parking is available at the main entrance of the building.

    Residency Program

    Resident and fellow physicians are licensed physicians participating in a formal postgraduate training program to gain advanced skills and knowledge. Residents and fellows care for patients within their training program’s specialty. At Baptist Health, residents and fellows work collaboratively with faculty physicians and allied health professionals to ensure the best patient outcomes.
