Preparing You For Your Visit

To prepare for your visit, please make sure to bring the following items with you:

  • Photo identification
  • Insurance card
  • Copies of any relevant medical records, radiology images, consultation reports, etc.
  • Name of pharmacy, full address, and phone number, in the event that a prescription is ordered

    • Parking

    Dedicated parking spots are available curbside in front of the practice. There are also designated parking spots clearly indicated on levels 2 and 3 of the parking garage. The parking garage entrance is located on Aurora Street. The parking ticket machine provides tickets for patients and visitors. Parking tickets are validated at the front desk of the primary care practice.

    We understand when you’re sick or in pain, you want to feel better as quickly as possible. That’s why we’ve included Baptist Health Pharmacy services at this location. Meds2Home provides our patients convenience and expertise without the additional trip to an outside pharmacy. We work closely with your physician and insurance provider to get the medications you need when you need them. Our pharmacy team also ensures all medications are in accordance with your treatment plan, providing you with a seamless, integrated healthcare experience.
