Parking is easily accessible near the hospital’s main entrance on SW 62 Place. You can also use our shuttle service that goes to and from the hospital with a stop at the South Miami Metrorail Station. Call 786-662-4199 to request shuttle services.

Upon arrival, please have your photo ID and insurance card ready as well as any medical records or forms requested by our orthopedic team. It is helpful to have contact information for your preferred pharmacy as well.

To prepare for your visit, please make sure to bring the following items with you:

Photo identification

Insurance card

Written authorization of referral from insurance company (If applicable)

Completed history and physical questionnaire and signed consent forms (If applicable)

Copies of any relevant medical records, radiology images, consultation reports, etc.

Name of pharmacy, full address, and phone number, in the event that a prescription is ordered
