Garage parking is easily accessible off U.S. 1. Please make sure to have your photo ID and insurance card at your appointment as well as any requested medical records. Having contact information for your preferred pharmacy including name, phone number and address is helpful as well.

Our team includes board-certified orthopedic surgeons, physiatrists and primary care sports medicine physicians who are ready to get you back to what you love.

Our 34 board-certified, fellowship-trained orthopedists have the expertise to diagnose and treat a wide variety of bone and joint problems. From fractures to muscle strains to complex musculoskeletal conditions, we offer compassionate, complete care to get you moving comfortably.

Preparing You For Your Visit

To prepare for your visit, please make sure to bring the following items with you:

  • Photo identification
  • Insurance card
  • Written authorization of referral from insurance company (If applicable)
  • Completed history and physical questionnaire and signed consent forms (If applicable)
  • Copies of any relevant medical records, radiology images, consultation reports, etc.
  • Name of pharmacy, full address, and phone number, in the event that a prescription is ordered
