To access the Medical Arts Building, use entrance J and then take West Access Road. Visitor parking is across from the Medical Arts Building entrance. Please bring your photo ID, insurance card and any requested records or forms with you. Our warm and friendly registration team will help you find the orthopedics department. It is also helpful to have the name, phone number and address for your preferred pharmacy.

Preparing You For Your Visit

To prepare for your visit, please make sure to bring the following items with you:

  • Photo identification
  • Insurance card
  • Written authorization of referral from insurance company (If applicable)
  • Completed history and physical questionnaire and signed consent forms (If applicable)
  • Copies of any relevant medical records, radiology images, consultation reports, etc.
  • Name of pharmacy, full address, and phone number, in the event that a prescription is ordered
