Our board-certified and fellowship-trained orthopedic physicians treat a wide variety of bone and joint problems. We see patients with dislocations, fractures, ligament and tendon tears, muscle strains, sprains and more. And if you’re a recreational or professional athlete, our team is here for you. Our experts include orthopedic surgeons who specialize in sports medicine, primary care sports medicine physicians, physician assistants, athletic trainers and physical therapists.

We aim to get you moving comfortably again – whether walking around your backyard or competing on the field.

To prepare for your visit, please make sure to bring the following items with you:

  • Photo identification
  • Insurance card
  • Healthcare surrogate or power of attorney (if applicable)
  • Written authorization of referral from insurance company (If applicable)
  • Completed history and physical questionnaire and signed consent forms (If applicable)
  • Copies of any relevant medical records, radiology images, consultation reports, etc.
  • Name of pharmacy, full address, and phone number, in the event that a prescription is ordered
