Meet the Team

Led by medical director Ronald Tolchin, D.O., our team includes Board-certified physiatrists who specialize in physical medicine and rehabilitation treatments for neurological conditions. We also work closely with physical therapists, occupational therapists, nutritionists, psychologists, nurses and physician assistants who are highly skilled in treating neurological injuries, musculoskeletal and related conditions.

Preparing You For Your Visit

To prepare for your visit, please make sure to bring the following items with you:

  • Photo identification
  • Insurance card
  • Written authorization of referral from insurance company (If applicable)
  • Completed history and physical questionnaire and signed consent forms (If applicable)
  • Copies of any relevant medical records, radiology images, consultation reports, etc.
  • Name of pharmacy, full address, and phone number, in the event that a prescription is ordered
