Expert general surgeons from our extensive Baptist Health network of care is conveniently located at the Somerset Shoppes on the corner of Glades and Lyons Road.

Warm, welcome and inviting — our goal is to give every patient excellent care, every time. Hernias, diverticulitis, GERD and diseases of the gastrointestinal tract are all treated here in Boca Raton with the same level of care you would expect at a larger facility.

Please have your photo ID and insurance card ready when you arrive and our warm and welcoming team of experts will be ready to help. If you have a preferred pharmacy, please let our team know the name and address as well.

Preparing You For Your Visit

To prepare for your visit, please make sure to bring the following items with you:

  • Photo identification
  • Insurance card
  • Written authorization of referral from insurance company (If applicable)
  • Completed history and physical questionnaire and signed consent forms (If applicable)
  • Copies of any relevant medical records, radiology images, consultation reports, etc.
  • Name of pharmacy, full address, and phone number, in the event that a prescription is ordered
