You can use our convenient parking garage next to Baptist Health South Miami Hospital on SW 62 Place. Feel free to contact our shuttle service at 786-662-4199 to request shuttle service to and from the hospital with a stop at the South Miami Metrorail station.

Registration is quick and easy when you bring your photo ID and insurance card with you. If our brain and spine care team asked for any medical records or if your insurance company needs any insurance forms completed, please bring those as well. In addition, bring the name, phone number and contact information for your preferred pharmacy. To prepare for your visit, please make sure to bring the following items with you:

Photo identification

Insurance card

Written authorization of referral from insurance company (If applicable)

Completed history and physical questionnaire and signed consent forms (If applicable)

Copies of any relevant medical records, radiology images, consultation reports, etc.

Name of pharmacy, full address, and phone number, in the event that a prescription is ordered
