From SW 94 Street, use Entrance J and continue on West Access Road. Visitor parking is opposite the entrance to the Medical Arts Building. Bring your photo ID, insurance card and any requested records or forms. It is also helpful to have the name, address and phone number of your preferred pharmacy. Our warm and welcoming registration staff will guide you to the orthopedics department.
