Our multi-disciplinary team provides care within your home for chronic disease management, recovery from an acute illness or to provide rehabilitative therapy. Team members visit your home in person or connect with you remotely through our Telehealth Center, which includes a tele-pharmacy to answer your medication questions.

You don’t have to be a Baptist Health patient or recently hospitalized to use Home Care. We offer services in Miami-Dade and Monroe Counties and some surrounding areas on referral from regional hospitals, community physicians, short-term rehabilitation facilities, skilled nursing and assisted living facilities.
