Your health is our primary concern – and yours. As your physician’s partner in your care, you are responsible for providing us with the complete health information we need to ensure safe treatment and for following care and treatment plans prescribed by your doctor before and after surgery.

Please review the following information designed to help us safeguard your health and well-being. 

  • Notify your physician immediately if you develop any changes in your health between your initial visit to your physician and the day of your procedure. Please report even minor changes, including a cough, cold or elevated temperature. If you suspect that you are pregnant, please notify your physician.
  • If you are taking medications for high blood pressure, diabetes or heart problems, your physician will give you instructions about taking the medications the day of your surgery. Please follow those instructions.
  • If your procedure requires sedation, you must be accompanied by a responsible adult to drive you home.

  • Payment and Insurance

    A patient financial representative will contact you approximately five days prior to your procedure. He or she will review your financial responsibility. Payment is due on the date of your procedure. When you visit Baptist Eye Surgery Center for your first appointment, our staff will help you begin the process of preparing for surgery, including establishing payment. Please bring your insurance and identification cards with you on the day of your procedure. If you have executed an Advance Directive, Health Care Surrogate or Power of Attorney, please bring a copy for our records.

    Our patient care representatives are here to assist with your insurance or billing questions. Please call our business office at 954-572-5888 with any questions you may have.

    Financial responsibility rests with the patient for deductibles, coinsurance and non-covered services. Your insurance company can also help you determine your coverage and answer any questions you may have regarding your plan policy. In addition to personal checks, we accept Visa, MasterCard, American Express and debit cards. Please remember to bring your insurance card, driver’s license, and a form of payment with you. ​
