Sohemi Pagan Leon, MD

Sohemi Pagán León, M.D., is an internal medicine physician at Baptist Health Primary Care.

She provides comprehensive primary care services for adults, including preventive care, wellness visits, chronic disease management and coordination of care. Her clinical interests include healthy aging, cognitive health, dementia care and helping patients maintain their independence and quality of life as they age.

Prior to joining Baptist Health, Dr. Pagán León completed a fellowship in geriatric medicine at the Johns Hopkins University School of Medicine, where she cared for older adults in outpatient, home-based and long-term care settings. She also served as an associate medical director during a clinical leadership rotation at Moorings Park Continuing Care Retirement Community in Naples, Florida.

Dr. Pagán León earned her medical degree from Ponce Health Sciences University School of Medicine in Puerto Rico. She completed an internal medicine residency at Johns Hopkins Bayview Medical Center and a fellowship in geriatric medicine at the Johns Hopkins University School of Medicine.

She is a member of the American Geriatrics Society and Alpha Omega Alpha Honor Medical Society. Dr. Pagán León is fluent in English and Spanish.

Dr. Pagán León is committed to delivering compassionate, patient-centered care. She works closely with patients and their families to develop individualized treatment plans that reflect each patient's goals, values and health needs.

When she is not treating patients, Dr. Pagán León enjoys spending quality time with her family and friends. She also enjoys cooking, taking walks with her dog and traveling.
