What Happens to Your Heart When You Sit Too Long

Most people know that regular exercise is important for heart health. But growing research suggests that what you do during the rest of the day matters, too. Spending long hours sitting—whether at a desk, in a car or on the couch—has been linked to an increased risk of heart disease, even among people who meet recommended exercise guidelines.
Researchers now describe prolonged sitting as a separate health risk from simply not exercising. In other words, going to the gym for 30 or 60 minutes does not necessarily erase the effects of remaining sedentary for the other 10 to 12 waking hours.
For that reason, heart specialists increasingly encourage people to reduce the amount of time they spend sitting and to incorporate short movement breaks throughout the day.
"Even if you exercise regularly, prolonged sitting can have a detrimental impact on your cardiovascular health and circulation," says Michele L. Taubman, M.D., vascular and endovascular surgeon at Baptist Health Miami Cardiac & Vascular Institute, in a recent Baptist Health Instagram reel.
Why Sitting for Hours Is a Concern
The human body was designed to move. Walking, climbing stairs and even standing require muscles to contract, helping blood circulate efficiently throughout the body. When movement stops for long periods, those natural processes slow down.
Sitting for extended periods can reduce blood flow in the legs, contribute to blood vessel dysfunction and make it easier for blood to pool in the lower extremities. Over time, these changes may contribute to cardiovascular problems.
Research published in recent years has consistently found that prolonged sedentary behavior is associated with a higher risk of heart attack, stroke, heart failure and premature death from cardiovascular disease. Studies also have linked excessive sitting to obesity, type 2 diabetes and metabolic syndrome—conditions that further increase the risk of heart disease.
"Many people think that working out once a day is all they need to do, but what happens during the rest of the day when you're inactive?” Dr. Taubman says. “Prolonged sitting has been linked long-term with high blood pressure, poor circulation, and negative impact on cardiovascular disease."
High blood pressure, also called hypertension, forces the heart to work harder to pump blood throughout the body. Poor circulation refers to reduced blood flow, particularly to the arms and legs, which may cause swelling, discomfort or fatigue.
What Recent Research Shows
Scientific evidence supporting regular movement continues to grow.
A 2024 study published in the Journal of the American College of Cardiology found that adults who spent more than about 10.5 hours each day being sedentary had significantly higher risks of heart failure and cardiovascular death, even if they met recommended levels of moderate-to-vigorous physical activity. The findings suggest that prolonged sitting carries its own cardiovascular risks that exercise alone may not completely eliminate.
Other recent research has shown that replacing even small amounts of sitting with light physical activity—such as walking, standing or household chores—can improve blood sugar control, lower blood pressure and reduce cardiovascular risk factors.
The American Heart Association also recommends reducing sedentary time throughout the day, noting that frequent movement benefits both heart health and overall well-being.
What Happens Inside Your Body
One of the earliest effects of prolonged sitting involves circulation.
Normally, muscles in the legs contract with every step you take. Those contractions squeeze nearby veins, helping push blood back toward the heart. When you remain seated for hours, that pumping action slows considerably.
Blood can begin pooling in the lower legs, increasing pressure within the veins. This sluggish circulation may contribute to leg swelling, discomfort and, in some people, varicose veins. In rare cases, especially during prolonged travel or periods of immobility, slow blood flow can increase the risk of developing a deep vein thrombosis (DVT), a blood clot that forms in a deep vein.
Beyond circulation, extended sitting also affects the lining of blood vessels, known as the endothelium. Healthy blood vessels expand and contract to maintain normal blood flow. Research suggests prolonged inactivity can impair this function, contributing over time to the development of atherosclerosis, the buildup of fatty plaques inside arteries.
Sitting for long periods also reduces calorie burning and can make it harder for the body to regulate blood sugar and cholesterol levels. These changes may contribute to weight gain, insulin resistance and elevated cholesterol—all established risk factors for cardiovascular disease.
Small Movement Breaks Can Make a Big Difference
Fortunately, you do not need to run a marathon to offset some of the effects of sitting.
Even brief periods of movement can improve circulation and help keep blood flowing through the legs.
"Even simple movements throughout the day can improve your circulation. Think about setting an alarm once an hour if you're playing video games or you have a desk job. Once that buzzer goes off, get up, walk around, take a break," Dr. Taubman says.
Research has found that interrupting sitting with short walks or standing breaks every 30 to 60 minutes can improve blood flow, lower blood sugar after meals and reduce stiffness.
These movement breaks may be especially important for people who work at computers, spend long hours driving or frequently watch television for extended periods.
The Power of the Calf Muscle Pump
One of the body's most effective tools for maintaining healthy circulation is located in the lower leg.
The calf muscles are often called the body's "second heart" because each contraction helps push blood upward through the veins toward the heart. This mechanism is known as the calf muscle pump.
When you're walking, climbing stairs or even rising onto your toes, these muscles squeeze the veins and assist blood return against gravity.
Dr. Taubman explains why getting up regularly is so effective.
"That's going to activate your calf muscle pump in your leg and help promote venous blood return,” she says. “Consider getting a standing desk that can go up and down. And again, do those laps during the day. Even just for a minute, every hour can really make a difference."
A standing desk can help reduce sitting time, but experts note that standing all day is not the answer either. Alternating between sitting, standing and walking appears to provide the greatest benefit.
Easy Ways to Move More Every Day
Reducing sedentary time does not require major lifestyle changes. Small habits performed consistently can improve circulation and support long-term cardiovascular health.
Helpful strategies include:
- Set a timer to stand and move every hour.
- Walk while talking on the phone.
- Take the stairs whenever possible.
- Park farther away from building entrances.
- Walk around during television commercials.
- Stretch or march in place during work breaks.
- Alternate between sitting and standing if using a height-adjustable desk.
- Take short walks after meals to promote circulation and blood sugar control.
These activities may seem minor individually, but together they can substantially reduce the amount of uninterrupted sitting throughout the day. As Dr. Taubman emphasizes, small actions performed consistently can have a meaningful impact on a person’s health.
