Water First, Coffee in Moderation: What to Drink for Better Cognitive Health

When people think about protecting their brain as they age, they often focus on puzzles, exercise or healthy foods. But what they drink every day may also play an important role.
According to G. Peter Gliebus, M.D., chief of neurology and director of Cognitive and Behavioral Neurology at Marcus Neuroscience Institute, part of Baptist Health, at Boca Raton Regional Hospital, the single best beverage for cognitive health is surprisingly simple: water.
While coffee has attracted growing scientific interest for its potential brain benefits, Dr. Gliebus emphasizes that hydration remains the foundation of healthy brain function. Coffee may provide a helpful boost and could be associated with long-term cognitive benefits, but it should complement—not replace—other healthy lifestyle habits.
Why Hydration Matters for the Brain
The human brain is about 75 percent water, making adequate hydration essential for normal function. Even relatively small fluid losses can interfere with mental performance.
"The best drink to support cognitive health is water," says Dr. Gliebus. "The brain depends on proper hydration to function well, and even mild dehydration can affect focus, alertness, memory, mood, and overall mental performance."
Dehydration occurs when the body loses more fluids than it takes in. Even mild dehydration can reduce blood volume, making it harder for oxygen and nutrients to reach the brain efficiently.
"Water helps support blood flow, nutrient delivery, and temperature regulation, all of which are important for healthy brain function," Dr. Gliebus explains.
Blood flow is especially important because it delivers oxygen and glucose—the brain's primary fuel—to billions of nerve cells. Without adequate hydration, these processes become less efficient, potentially affecting thinking and memory.
Research also suggests that chronic poor hydration may contribute to worsening cognitive performance over time, particularly among older adults, whose sense of thirst naturally declines with age.
"While drinks like coffee and tea may offer short-term alertness benefits, water should be the foundation," Dr. Gliebus says. "Staying well hydrated throughout the day is one of the simplest ways to support clearer thinking, better concentration, and overall brain health."
Coffee's Brain Boost
Coffee remains one of the world's most popular beverages, and scientists continue to study how it affects brain health.
According to Dr. Gliebus, moderate coffee consumption can provide measurable short-term improvements in mental performance.
"Coffee can have a positive effect on cognitive health when consumed in moderation," he says. "The caffeine in coffee stimulates the central nervous system and can temporarily improve alertness, attention, reaction time, and the ability to concentrate."
The central nervous system includes the brain and spinal cord, which control thought, movement, memory and many other body functions.
Caffeine works by blocking adenosine, a naturally occurring brain chemical that promotes sleepiness throughout the day. As adenosine levels build, people feel increasingly tired. By temporarily blocking its effects, caffeine helps people feel more awake and mentally alert.
For many people, this translates into improved concentration, faster decision-making and better attention during mentally demanding tasks.
However, these effects are temporary and should not be confused with long-term protection against cognitive decline.
What the Research Shows About Coffee and Dementia
In recent years, researchers have increasingly examined whether regular coffee consumption may influence the risk of developing dementia and other neurodegenerative diseases.
A large study published in February involved more than 130,000 participants followed for decades. Researchers found that people who regularly consumed approximately two to three cups of caffeinated coffee daily had a lower risk of developing dementia and experienced slightly better cognitive function than non-coffee drinkers. Notably, the association was seen with caffeinated beverages but not decaffeinated coffee.
Other recent observational studies have reported similar findings, suggesting moderate coffee consumption may be associated with a lower risk of Alzheimer's disease, Parkinson's disease and cognitive decline, although the exact mechanisms remain under investigation.
Researchers believe coffee's potential benefits may stem from more than caffeine alone. Coffee contains hundreds of naturally occurring compounds, including antioxidants and polyphenols, which may help reduce inflammation and oxidative stress—two biological processes believed to contribute to aging and neurodegenerative diseases.
Still, experts caution that these studies show an association, not proof that coffee prevents dementia. People who regularly drink moderate amounts of coffee may also engage in other healthy behaviors that contribute to better brain health.
"There is also growing research suggesting that moderate coffee consumption may be associated with better long-term brain health," Dr. Gliebus says. "Some studies have linked caffeinated coffee intake with a lower risk of cognitive decline, dementia, Parkinson's disease and Alzheimer's disease in certain groups, though coffee should not be viewed as a guaranteed way to prevent these conditions."
Moderation Is Key
More coffee is not necessarily better. Dr. Gliebus stresses that moderation is essential because excessive caffeine can produce unwanted side effects that may ultimately interfere with brain health.
"The key is moderation," he says. "Too much caffeine can cause jitteriness, anxiety, heart palpitations, dehydration if it replaces water intake, and sleep disruption, and poor sleep can negatively affect memory and cognitive function."
Heart palpitations are sensations that the heart is beating unusually fast, hard or irregularly. While often harmless, they can be uncomfortable and may require medical evaluation in some individuals.
Sleep disruption deserves particular attention because sleep plays a critical role in memory formation and brain repair. During deep sleep, the brain strengthens newly learned information and clears away metabolic waste products. Chronic sleep loss has been linked to poorer memory, impaired attention and increased dementia risk.
Most healthy adults can safely consume up to about 400 milligrams of caffeine daily—roughly the amount found in four 8-ounce cups of brewed coffee—but individual tolerance varies. People with heart conditions, anxiety disorders, pregnancy or sleep problems may need significantly less caffeine.
Brain Health Requires More Than One Beverage
Although coffee may be part of a healthy lifestyle, Dr. Gliebus says no single beverage can replace the habits that have consistently been shown to support long-term cognitive health.
Research consistently shows that protecting brain health requires a comprehensive approach. Regular physical activity improves blood flow to the brain. A Mediterranean-style eating pattern rich in fruits, vegetables, whole grains, fish and healthy fats has been associated with slower cognitive decline. Managing high blood pressure, diabetes and cholesterol also helps reduce dementia risk, as does staying socially engaged and mentally active.
"For most people, coffee is best used as a helpful boost, not a replacement for water, sleep, exercise, a healthy diet, and other habits that protect brain health," Dr. Gliebus says.
