Takeaways from President Trump’s Chronic Venous Insufficiency Diagnosis

When President Donald Trump, age 79, was recently diagnosed with chronic venous insufficiency (CVI), it highlighted a common — yet often overlooked  health condition. This diagnosis underscores the importance of recognizing potential warning signs, and seeking prompt medical evaluation, especially for older adults or those with sedentary lifestyles.

CVI occurs when valves inside the veins in the lower legs aren’t functioning properly. These valves are responsible for ensuring blood flows back toward the heart. When they fail to work as efficiently as they should, blood can pool in the veins, leading to symptoms such as:

  • Swelling in the lower legs or ankles.
  • Pain, cramping, or aching in the legs.
  • Varicose veins.
  • Changes in skin color or texture around the affected area.

Chronic Venous Insufficiency can lead to swelling of the legs -- and in very rare cases -- can increase the risk of a blood clot in the leg,” explains Nicholas Cortolillo, M.D., a vascular surgeon with Baptist Health Heart & Vascular Care. “The first-line treatment for all patients with chronic venous insufficiency is what we call graduated compression stockings.”

According to a statement from the White House, President Trump sought medical attention after “noting mild swelling in his lower legs” over a span of weeks. A comprehensive evaluation, including diagnostic vascular studies, was conducted by the White House Medical Unit. His physician, Capt. Sean Barbabella, confirmed the diagnosis, describing CVI as “a benign and common condition, particularly in individuals over the age of 70.”

Dr. Cortolillo adds that CVI is fairly common but “it's a very under-diagnosed condition,” affecting about 20 percent of U.S. adults. “The undiagnosed category is upwards of an additional 10 percent -- about one in three Americans has this condition.”

He emphasizes the importance of giving compression stockings a good try.

“These are custom-fitted compression stockings that cover the ankle and calf to the knee and to the thigh,” explains Dr. Cortolillo. “And we set a pressure of around 20 to 30 millimeters of mercury -- just barely enough to keep the blood flowing in the correct direction back to the heart. And this alleviates the majority of symptoms in the majority of patients.”

While CVI itself is not life-threatening, it can have a significant impact on mobility and quality of life, especially if left untreated. The risk increases with age, and according to the letter from President Trump’s physician, the condition is “not surprising” in older individuals, particularly those with additional risk factors.

Constantino Pena, M.D., an interventional radiologist at Baptist Health, explains that seeking medical care is the first step toward alleviating any concerns. “The earlier you get evaluated, the sooner we can secure that diagnosis and identify potential issues to help prevent more serious complications,” said Dr. Pena. “CVI is a common and very manageable condition for most individuals.”

Why Symptoms Shouldn’t Be Ignored

Though CVI may seem like a straightforward condition, experts emphasize the importance of considering other potential underlying causes. Venous insufficiency might point to other health issues, such as increased pressure in the heart or lungs, which could contribute to the problem.

Dr. Cortolillo emphasizes not to dismiss swelling, discomfort, or other unusual changes in your legs as just aging.  Recognizing symptoms early and consulting a healthcare provider can help prevent complications and improve outcomes. The CVI diagnosis should not impede President Trump’s duties, he adds.

“This isn't something that would interfere with his ability to carry out the duties of the president,” said Dr. Cortolillo. “With the frequent flying that the White House requires, he is at a slightly higher increased risk of what's called deep vein thrombosis (DVT) in the lower extremity. But he shouldn’t have any issues with proper prevention and proper mitigating measures.”

DVT, or Deep Vein Thrombosis, is a condition where a blood clot (thrombus) forms in a deep vein, most commonly in the legs. Sitting for prolonged periods of time is a risk factor.

High-Risk Groups and Preventive Steps

CVI disproportionately affects certain groups, including older adults, individuals with obesity, those leading primarily sedentary lifestyles and those who stand for prolonged periods of time. If you fall into a high-risk group, consider the following steps to maintain better vascular health:

Stay Active: Regular movement, whether through walking, cycling, swimming, or even light stretching, helps promote healthy blood flow. Aim for at least 30 minutes of moderate activity a day.

Elevate Your Legs: If you experience leg swelling or spend extended periods sitting or standing, taking short breaks to elevate your legs above heart level can reduce pooling in the veins.

Maintain a Healthy Weight: Excess weight strains your veins and increases the likelihood of developing CVI. A balanced diet paired with exercise is key to managing or preventing obesity-related complications.

Wear Compression Stockings: Compression garments provide extra support to your veins, helping to improve circulation and minimize swelling.

Hydrate and Avoid Prolonged Periods Without Movement: Sitting for long hours increases the risk of vein-related issues. Try to stand up and stretch every hour, especially if your work involves a desk or you're traveling.

Seeking Proper Care

CVI itself isn’t life-threatening, but it can lead to discomfort and decreased mobility. Many conditions can cause leg swelling, making diagnosis important.

While President Trump’s doctors found no serious complications, his evaluation reflects the importance of thorough medical investigation. If you experience similar symptoms — such as swelling, discoloration, or cramping — don’t hesitate to consult a healthcare provider.
