Heart Damage from Diabetes Can Develop Quietly Over Time

Millions of Americans live with diabetes, carefully monitoring their blood sugar and managing their diet. But many may not realize that while they're focused on glucose levels, their heart could be quietly taking damage — long before any warning signs appear.

"Diabetes can affect your heart long before you feel any symptoms," said Christina Michael, M.D., cardiologist and medical director of the Echo Lab at Baptist Health Heart & Vascular Care at Bethesda Hospital East. "Many people don't realize how closely diabetes and heart health are connected. Because symptoms are not always obvious early on, heart damage can develop quietly over time."

That silent progression is precisely what makes the diabetes-heart disease connection so dangerous — and so important to understand.

The Hidden Link Between Blood Sugar and Heart Health

Diabetes is a chronic condition in which the body either doesn't produce enough insulin or can't use it effectively, leading to elevated levels of glucose (sugar) in the bloodstream. Most people associate it with complications like nerve damage or kidney disease. But the cardiovascular system — the heart and network of blood vessels that carry blood throughout the body — is also a primary target.

"High blood sugar can damage blood vessels and cause hardening of arteries, which can increase the risk of heart disease and stroke," Dr. Michael explained in a recent Baptist Health Instagram reel.

The "hardening of arteries" she describes is a condition called atherosclerosis, in which fatty deposits and scar tissue build up along artery walls, making them stiffer and narrower over time. This forces the heart to work harder to pump blood through constricted passages, raising the risk of heart attack and stroke — the leading causes of death among people with diabetes.

What makes it especially insidious is the gradual nature of the process. Artery damage can accumulate over years without producing any noticeable symptoms, leaving many people unaware that anything is wrong until a serious cardiac event occurs.

Circulation, Energy, and the Cardiovascular Ripple Effect

The consequences of poorly controlled blood sugar extend well beyond the arteries. Dr. Michael noted that over time, high glucose levels "can impact circulation, energy levels, and overall cardiovascular health, even if you're not feeling anything yet."

Reduced circulation — the movement of blood to the body's tissues and organs — can affect nearly every system in the body. The heart muscle itself may not receive adequate blood flow, weakening its ability to pump efficiently. This condition, known as coronary artery disease, is significantly more common in people with diabetes than in the general population.

Fatigue and low energy, symptoms that are easy to dismiss or attribute to stress or aging, can also be early indicators that the cardiovascular system is under strain. Because these signs are subtle and nonspecific, they're frequently overlooked by both patients and, in some cases, clinicians who aren't specifically screening for cardiac involvement.

Diabetes can also cause a condition called diabetic cardiomyopathy — a disease of the heart muscle itself, separate from artery damage — in which the heart becomes stiff or weakened even in the absence of blocked arteries. Researchers believe that chronically high blood sugar directly injures heart muscle cells and disrupts their function over time.

Why Early Management Is the Best Defense

Given that heart damage can progress silently, Dr. Michael emphasizes that waiting for symptoms before taking action is a losing strategy.

"Understanding this connection is important because managing your diabetes early can protect your heart even before complications develop," she said.

That protective window — the period before structural heart damage becomes established — is when lifestyle interventions and medical management have the greatest impact. The American Heart Association and American Diabetes Association both recognize that tight blood sugar control in the early stages of diabetes is strongly associated with lower rates of cardiovascular complications later in life.

"Taking control early by eating a healthful diet, exercising, and maintaining a normal weight can help improve your long-term heart health," Dr. Michael said.

Practical Steps to Protect Your Heart

For people living with diabetes or prediabetes — a state in which blood sugar levels are elevated but not yet high enough to qualify as full diabetes — Dr. Michael's recommendations translate into a handful of concrete daily habits.

Diet plays a foundational role. A heart-healthy eating pattern emphasizes vegetables, whole grains, lean proteins, and healthy fats while limiting processed foods, refined sugars, and excess sodium. The goal is not only to keep blood sugar stable but also to reduce inflammation and cholesterol levels that contribute to artery damage.

Regular physical activity is equally important. Exercise improves the body's sensitivity to insulin (meaning it takes less insulin to manage blood sugar), lowers blood pressure, reduces inflammation, and directly strengthens the heart muscle. Even moderate activity — such as 30 minutes of brisk walking most days of the week — has been shown to reduce cardiovascular risk in people with diabetes.

Weight management matters because excess body fat, particularly around the abdomen, is associated with insulin resistance — a condition in which the body's cells don't respond normally to insulin, causing blood sugar levels to rise. Losing even a modest amount of weight, five to ten percent of body weight in some studies, can meaningfully improve blood sugar control and reduce strain on the heart.

Routine medical care is also essential. Regular check-ups allow physicians to monitor blood sugar (through a test called hemoglobin A1C, which reflects average glucose levels over roughly three months), blood pressure, and cholesterol — all of which factor into cardiovascular risk. In some cases, medications specifically designed to protect the heart in people with diabetes may be recommended.
