The Diabetic Hand: Managing Conditions Such as ‘Trigger Finger’ and ‘Stiff Hand Syndrome’

When one thinks about the complications of diabetes, conditions affecting the hands are not at the forefront. However, long-standing or poorly controlled blood sugar can also take a quiet but significant toll on the hands.

In a recent webinar by Baptist Health International, Roy Cardoso, M.D., an orthopedic hand and upper extremity surgeon with Baptist Health Orthopedic Care, shed light on a group of conditions collectively known as the diabetic hand. According to Dr. Cardoso, diabetes drastically increases your risk for structural, nerve, and joint issues in the upper extremities.

Understanding how diabetes changes the tissue in your fingers and palms can help individuals spot early warning signs and seek timely treatment before daily functionality is compromised.

The Root Cause: How Sugar Stiffens the Hand

To understand why the hand is so vulnerable to diabetes, it helps to look at what is happening at a microscopic level. Dr. Cardoso explains that chronic hypoglycemia (consistently high blood sugar levels) triggers a chemical reaction in the body that produces Advanced Glycation End-products (AGEs).

These AGEs cause abnormal cross-linking in collagen, the primary structural protein found in your skin, tendons, and joints. Dr. Cardoso uses a relatable analogy from the kitchen. "This creates a Maillard reaction,” Dr. Cardoso said. “If you're a chef, you're familiar with the Maillard reaction because this is what makes your meat brown. You want it if you're cooking steak, but not so much in your hand."

This biological "browning" or stiffening process makes the collagen in your hands thick, inelastic, and highly resistant to being naturally broken down and repaired by the body. At the same time, microangiopathy — damage to the body's tiniest blood vessels — restricts healthy blood flow to the hand's skin, tendons, and nerves. The result is a combination of rigid tissues and starved nerves that sets the stage for several distinct hand conditions.

1. Trigger Finger (Stenosing Tenosynovitis)

Trigger finger is one of the most talked-about and frequently seen conditions in a hand surgeon's clinic, and its connection to diabetes is incredibly strong.

What it is:

Your fingers bend and straighten thanks to long, rope-like structures called tendons that slide through a series of protective tunnels or rings known as pulleys. When diabetes causes collagen to stiffen, the tenosynovium (the protective sleeve around the tendon) thickens and swells. This creates a spatial mismatch: the swollen tendon can no longer glide smoothly through the rigid pulley tunnel. The finger can catch, pop, lock, or become painfully stuck in a bent position.

The Diabetic Difference:

While a non-diabetic individual might develop a single trigger finger from an isolated injury or repetitive strain, diabetic patients face unique challenges. "One of the things that I see in my practice is someone with diabetes who doesn't have one trigger finger — they have like four trigger fingers,” Dr. Cardoso explains. “If you see a patient who has multiple trigger fingers, we always look for a reason."

Individuals with diabetes have a two to nine times higher risk of developing trigger finger than the general public, with type 1 diabetics seeing the highest prevalence due to the duration of their disease.

Treatment Options:

  • Cortisone Injections: A standard anti-inflammatory steroid injection is typically the first line of defense. However, Dr. Cardoso notes that injections are less effective in insulin-dependent patients, providing long-term relief only about 40 percent of the time. Furthermore, a steroid injection can cause blood glucose spikes of 50 to 150 mg/dL that last 24 to 72 hours, meaning patients must closely monitor their blood sugar post-injection. Due to the risk of tendon rupture, hand specialists will rarely inject the same finger more than twice.

  • Surgery: If injections fail, a quick surgery called an open release can be performed under local anesthesia while the patient is awake. The surgeon makes a tiny incision to cut the tight pulley tunnel, giving the tendon room to move freely. Though highly successful, diabetic individuals have a slightly higher rate of minor post-operative stiffness and a 10 percent higher rate of recurrence compared to non-diabetics.

2. Diabetic Cheiroarthropathy (Stiff Hand Syndrome)

Derived from the Greek words cheiro (hand) and arthropathy (joint disease), this condition is also widely referred to as Limited Joint Mobility (LJM).

Symptoms and Signs:

Diabetic cheiroarthropathy is a progressive, painless stiffness of the hands. The skin on the back of the hand and the palms becomes thick, tight, and waxy. As it advances, patients experience mild flexion contractures, meaning their fingers naturally curl inward and cannot be forced flat.

Doctors screen for this using two simple visual assessments:

  1. The Prayer Sign: The patient is asked to press their palms together as if praying. If the palms and finger joints cannot completely touch, leaving visible gaps, the test is positive.
  2. The Tabletop Test: The patient attempts to place their hand completely flat on a flat surface. If the hand hinges up or fingers stay bent, it indicates limited mobility.

Treatment:

Because this is a systemic tissue issue rather than a structural blockage, surgery is rarely indicated and almost never works to restore movement. Instead, management relies on optimizing blood sugar levels to slow progression, performing daily hand-stretching exercises, utilizing heat therapies, and treating any coexisting conditions like carpal tunnel to preserve overall hand function.

3. Dupuytren’s Disease

Often historically referred to as "Viking’s Disease" due to its high prevalence in people of Scandinavian descent, Dupuytren's disease involves a thickening of the fascia—the layer of tissue just beneath the skin of your palm. Dr. Cardoso frequently uses a lighthearted story to explain its genetic roots to patients:

In diabetic individuals, Dupuytren's tends to present a bit differently than it does in idiopathic (spontaneous) cases. It is generally milder, forming firm bumps or nodules in the palm, but it has a less aggressive course and is less likely to severely bend the fingers over a short period.

Treatment:

Mild cases are simply observed. If a dense "cord" of tissue forms and limits movement, modern medicine offers non-surgical solutions like Xiaflex, a specialized enzyme injection. The doctor injects the collagenase enzyme into the tight cord to dissolve it, and the patient returns two days later to have the cord manually broken and straightened by the physician. While open surgery (fasciectomy) is an option, surgeons prefer non-invasive approaches for diabetic patients to avoid potential wound-healing complications.

4. Nerve Compression Syndromes

Because the fibrous tunnels in the wrist and elbow stiffen and narrow under the influence of diabetes, the nerves traveling through them frequently become pinched. This is compounded by nerve ischemia—poor blood flow directly to the nerve tissue itself.

  • Carpal Tunnel Syndrome: Compression of the median nerve at the wrist, causing numbness, tingling, and pain in the thumb, index, middle, and half of the ring finger.
  • Cubital Tunnel Syndrome: Compression of the ulnar nerve at the elbow, which presents as numbness or pins-and-needles in the ring and pinky fingers.

The risk of developing carpal tunnel syndrome is roughly three times higher for individuals with diabetes. Because the symptoms can closely mimic or overlap with diabetic neuropathy (generalized nerve damage in the extremities), hand specialists use detailed physical exams and sometimes nerve conduction studies to confirm exactly where the nerve is pinched.

When splinting or therapy does not solve the issue, Dr. Cardoso favors endoscopic carpal tunnel surgery. "I do endoscopic surgery because you get faster recoveries. Patients get back to work faster,” said Dr. Cardoso. “There are less wound problems ... The incision is fairly minimal with the endoscopic treatment."

The Takeaway for Patients

The hands can be a mirror of your systemic health. Dr. Cardoso points out that the total duration of a patient's diabetes plays a much larger role in developing these conditions than a temporary spike in blood sugar. However, maintaining an ideal Hemoglobin A1C (ideally in the 5 percent to 6 percent range) remains crucial, as poorly managed blood sugar dramatically increases the risk of post-surgical infections and slow wound healing.

Early intervention is critical to ensuring your recovery stays on track. Dr. Cardoso advises primary care providers and patients alike to seek specialized help sooner rather than later:

"Please refer early for patients with persistent trigger fingers or neuropathies because they don't typically get better as easily as do non-diabetic patients,” Dr. Cardoso explains. “Sometimes, we get these patients who are so stiff that they have had these issues for such a long time. And then it becomes very difficult to get them functional again."

If you notice multiple clicking fingers, a loss of the ability to lay your hands flat on a table, or persistent night-time numbness in your fingers, don't brush it off as simple aging, he adds. Early evaluation by an upper extremity specialist can pave the way for conservative therapies, preserving the strength, mobility, and freedom of your hands.
